The Senior Clinical Social Worker is a non-supervisory position that reports to the Chief of Mental Health or designee. The Senior Clinical Social Worker functions at the advance practice level providing clinical psychosocial and case management services to eligible veterans and their family members. Independently provides psychological and psychosocial services to Veterans in the Primary Care-Mental Health Integration program. The PCMHI Social Worker functions as part of a multidisciplinary team, and delivers brief, consultation-based services to Veterans, primary care providers, and allied Patient Alliance Care Team (PACT) members. The focus is on general service delivery for a wide range of concerns and resolving them within the primary care service context. Visits in PCMHI are brief, limited in number, and are provided in the primary care area, structured so that the patient views meeting with the psychologist as a routine primary care service. PCMHI Social Workers function within fast-paced primary care teams and can expect to consult with 8-10 patients per day with the primary goal of assisting PACT members with identification, treatment, and management of mental health conditions in the enrolled population. Provide supportive counseling and appropriate case management services to reduce the Veteran's stress throughout all phases of treatment. The incumbent incorporates complex multiple causation in differential diagnosis and treatment of Veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Duties include, but are not limited to: Provides brief mental health and behavioral interventions/assessments routinely, using time-limited, evidence-based interventions that are simple and concrete. Independently evaluates the client's situation, including the Veteran's reaction and ability to deal with it, and arrives at a reasoned conclusion. Serves as liaison for Charlie Norwood VA Medical Centers Mental Health Department in providing mental health services and case management services in a specialized program. Provides clinical services to Veterans and family members/significant others in support of the Veteran's treatment. Develops working relationships and agreements with other organizations having responsibilities for the same patient population. Establishes a continuing relationship with the Veteran, evaluating progress towards goals and adjusting the treatment plan as appropriate. Documents independently in the medical records utilizing paper and/or electronic formats, including psychosocial assessments, diagnostics, treatment, progress notes, follow up, and referral. Provides orientation and on-going training to other interdisciplinary team members. Provides clinical supervision for unlicensed/pre-doctoral psychologists. Provides training to students, residents, interns, and fellows from other disciplines. All other duties as assigned.
