Grand Itasca Clinic & Hospital, a part of Fairview Health Services, is a fully integrated clinic and hospital with over700 employees, including over 60 providers providing primary and specialty care. The hospital operates a 4-bed intensive care unit, 13-bed women's health and birth unit, 22-bed medical/surgical/pediatric unit, and a Level III Trauma Center emergency department that averages 13,000 visits on an annual basis. In the local and surrounding community, Grand Itasca is the primary provider of health care services. Grand Rapids is approximately 3 hours from Minneapolis, 90 min from Duluth.
In 2015, the Agency for Healthcare ResearchandQualitynamed Fairview Clinics as one of eight exemplar integrated behavioral health primary care models across the country. Integrated care is a result of an engaged primary care team and Behavioral Health Clinician providing patient-centered care addressingmental health and substance abuse conditions, health behaviors, stressors, social determinants of health, and ineffective patterns of health care utilization. We are currently hiring for the Behavioral Health Clinician (BHC) position, a key role within a multi-disciplinary team in the Integrated Behavioral Health model. We are looking for behavioral providers with strong diagnostic, clinical, team engagement, and patient care skills who enjoy innovation, variability in your work day, and being part of an exceptional team.
A Behavioral Health Clinician (BHC) is a fully integraged Mental Health Professional and core member of the primary care team that works under the direction of the primary care provider(s). The role of the BHC is to support the primary care team in the early identification and treatment of behavioral health conditions within the defined clinic population.
The primary funtions of the BHC are to:
Complete diagnostic evaluations
Provide consultation to the team regarding diagnostic impressions, recommendations, and strategies to engage patients in health and behavior change
Support the team with the day to day management of complex patients
Provide evidence-based, direct care to patients
Engage in population health activities to support risk stratified populations.
Masters or Doctoral Degree in Social Work or Psychology
Licensed Social Worker (LICSW) or Doctoral Level Licensed Psychologist (LP)
2 years post licensure
5 years postLICSW / LPlicensure
Previous experience completing diagnostic evaluations, and providing individual, couples, family counseling for children and adults
Knowledge of health conditions and common co-morbidities
Knowledge of the continuum of behavioral health care services (i.e. within surrounding communities)
