The incumbent in this position serves as the Supervisory Social Worker for Social Work and Community Care. The incumbent would be responsible for providing front line supervision to GS-185 staff located at the main facility and at 5 Community Based Outpatient Clinics located within the Medical Center's service territory. Included in this role are education, coaching, mentoring, work assigning and performance evaluation of line social work staff. Duties are divided into two categories. Direct Service Responsibilities as appropriate to clinical area of coverage: Conducting psychosocial assessments to determine needs of veterans and family members. Participating actively in the treatment planning process with other disciplines. Participating in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate. Providing individual, family and group counseling as appropriate. Providing consultation and education to veterans and families on psychosocial issues, community resources, advance directives and VA benefits. Acting as a consultant to other team members, professionals and administrative staff on social work practice issues.Serving as a liaison between VA and community resources, to include marketing and public relation duties as appropriate. Providing case management to veterans and families, as needed, throughout the continuum of care. Providing home assessments if appropriate. Supervising the practice of social work students, or a less experienced social worker. Maintaining data and statistical compilations to comply with Joint Commission, CARF, VA and medical center policy and procedures. Supervisory Social Worker Responsibilities: Serving as first line supervisor to 15-20 GS-185-9, 11 and 12 line social workers, including completing all administrative supervisory functions for oversight of the work of subordinate staff. Providing clinical supervision to line social work staff under supervision Providing mentoring and clinical supervision to social workers on social work practice issues. Reviewing staff workload reports for supervised staff and making work assignments based upon workload and clinical demand. Identifying and tracking Quality Improvement initiatives involving staff under direct supervision. Conducting mid-year and year-end performance evaluations of GS-185 line staff under supervision. Approval of T&L requests of subordinate line staff. Receiving and disseminating social work practice and resource information to social workers under supervision at the facility and all CBOC's. Assisting the Chief, Social Work and Community Care as needed to oversee clinical and administrative functions of the Social Work Department.
