Social worker serves as a member of a multidisciplinary Housing and Urban Development Veterans Affairs Supportive Housing (HUD VASH) team and is responsible for coordinating and providing individual and family counseling and case management services. The social worker; conducts assessment interviews, develops treatment plans and provides treatment to HUD VASH eligible Veterans and their families. Treatment modalities may include crisis intervention, intake evaluations, outreach services as well as empirically based treatment for family, couples, and mental health conditions. The social worker may refer clients to other programs and professionals as appropriate. Social Worker is responsible for providing HUD-VASH case management. Case management is defined as the provision of services by VA clinical staff to homeless veterans and includes all activities to provide appropriate treatment and maintain veterans in HUD approved stable housing. S/he will provide a full range of social work services within commonly accepted standards of social work practice which Includes case work, psychosocial assessments, individual, family and group counseling treatment planning, discharge planning, address Advance Directives, and other services as appropriate. The incumbent performs site visits of HUD-VASH apartments to ensure that veterans reside in safe environments in compliance with local housing codes; verifies the veteran's status, eligibility for VA medical care, and eligibility of program participation and is responsible for ensuring that homeless veterans are considered as primary participants for program acceptance. Social Worker coordinates care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery and linking and referring veterans to the VA Medical Center, VA Regional Offices and/or community agencies. TRAVEL REQUIRED: Occasional by car to travel to Veterans homes and or in the community to assist Veterans with housing and resources as needed.
