Incumbent serves as the Rehabilitation Therapy Referral Coordination Program Coordinator and is located within Minneapolis VA Health Care System. The Referral Coordination Initiative (RCI) is the Veterans Health Administrations process to streamline the referral process which will improve timeliness of care and empower veterans to understand the full range of care options. Major duties include, but are not limited to: Administrative/Program Management: Ability to apply advanced knowledge of contemporary physical therapy within specialty area to provide clinical guidance, training or education for internal and external stakeholder Demonstrates advanced knowledge of evidence-based practices and clinical practice guidelines in multiple professional areas, and the ability to use these resources to guide the provision of clinically appropriate treatment interventions across the continuum of care. First line evaluation and provision of therapy services in a direct access environment, mentorship of students and rehabilitation therapists, as needed coverage of outpatient orthopedic physical therapy services and evaluation of patients with specialized conditions/treatment protocols. Ability to serve as a consultant within the assigned program area for intra-agency and interagency planning and to provide service coordination to optimize program continuity, efficiency and effectiveness. Serves as a consultant and shares knowledge of RCI Rehabilitation Therapy implementation and development at a local and VISN level Serves as a resource to facility rehabilitation teams regarding community care processes Serves as a resource to facility rehabilitation teams on rehabilitation services available through other V23 facilities Knowledge of and skill in management, administration and/or education methodologies. This includes the ability to monitor and track data, utilize available resources effectively, evaluate program quality, and generate reports for local, and/or VISN and/or VACO leadership. Actively participates in program development and/or process improvements relevant to the RCI program to improve efficiency and quality of care provided to patients. Monitors outcomes, participates in strategic planning and implements strategies for program improvement Serves as a point of contact providing guidance to PM&R and facility leadership on matters related to RCI Ability to develop and coordinate treatment and/or educational programs within a concentrated field or specialty area, such as, but not limited to, amputee, spinal cord injury, chronic pain, student clinical education/residency programming, polytrauma/traumatic brain injury. Coordination of ongoing educational opportunities in (specialty area/s) for (facility) staff including shadowing as needed, in-services, and 1:1 mentoring Contributes to learning opportunities at the local level via conference planning, presentations at conferences, knowledge sharing, and consultation. Ability to establish and maintain collaborative relationships within department and with other departments or affiliated programs. Collaborates with physical therapy clinical specialists, PM&R and Primary Care service leaders to develop local policy for requesting physical therapy services through the Community Care program. Incumbent is part of a VISN wide Rehabilitation Therapy RCI Team that develops a system to facilitate cross coverage care across the VISN Ability to guide the work of a multi-disciplinary team.
