These positions are located at either the Honor Annex Community Based Outpatient Clinic or Linwood Community Based Outpatient Clinic or Linwood. Clinical duties include assuming primary medical responsibility for assigned panel of patients who will be primarily older adults with cognitive, functional and complex medical and/or social problems; identifying and defining the medical management of the patients' medical problems; and determining the need for consultation from medical, surgical, and/or psychiatric subspecialty clinics and other services, facilitating admission to the hospital when needed. Administrative duties include planning and directing the educational and clinical experience of medical students, residents, and fellows and other learners assigned to the GPC program.
